The report, based on interviews with local law enforcement, documents, and public testimony, highlighted a lack of communication between agencies that allowed the shooter, Thomas Matthew Crooks, to evade law enforcement and carry out the attack. The report detailed how law enforcement officers spotted Crooks with a firearm on the roof of a building but communication failures prevented the information from reaching the Secret Service in time. The report also noted that the Secret Service did not participate in crucial briefings with local law enforcement agencies prior to the rally. The task force is continuing to investigate the incident and is conducting interviews with federal officials to determine the full extent of the failures that led to the assassination attempt. Additionally, the panel is looking into a separate incident in which Trump was allegedly targeted at his golf course in West Palm Beach, Florida, on September 15. The Secret Service has not responded to requests for comment on the report.
